The Rapids Family Health Team Access to Care Centre continues to be open for acute episodic medical conditions, ie. cough, cold, flu symptoms, urinary tract infections, ear aches, rashes, etc.  If you are unwell and can’t get in touch with your primary care provider we are here to help.

Open:   Monday - Thursday    8 am – 4:30 pm,  Fridays   8 am - Noon,  Saturday 8 am - 1 pm                

Appointment required, please call 519-491-6188 or 519-491-5395 for a phone consultation.    Please be patient and kind the phone lines can be very busy. 

While on the phone you will be triaged by a Registered Nurse and then if appropriate transferred to a Nurse Practitioner.  If you need to be seen in person, arrangements will be made at that time.  

Located at 481 London Rd. (London Rd. @ Norman St., Sarnia) in the London Rd. Diagnostic Clinic - 1st floor beside the pharmacy.
