Receptionist

Part time position

Rapids Family Health Team is seeking an enthusiastic medical office receptionist (potential mobile position with travel between RFHT sites). As part of the interdisciplinary team, the Receptionist is the first point of contact for patients and visitors to our offices. This position performs administrative tasks, on a priority basis, that support the daily operations of Rapids Family Health Team.

Qualifications:

  • Successful completion of a Secondary school diploma required, Medical or Office Administration College diploma preferred
  • 2-3 years of experience in an administrative support position
  • Previous experience in a health care setting preferred

Registered Nurse (RN - Educator) – Foot Care

As a member of the Rapids Family Health Team, the RN Educator is a specialist in self-management, disease prevention and community development, and is responsible for the planning, coordination and delivery of services to specific populations.  The RN Educator will share expertise and knowledge of disease prevention and healthy living with clients and their families, as well as other health care providers.

This role is responsible for the following:

  • Determine and carry out appropriate advanced foot care nursing assessments, interventions and treatments according to the established highest quality care standards of practice including but not limited to diabetic foot care, nail care and wound care.
  • Understand and implement infection prevention practices while specifically adhering to care standards as it relates to foot care and diabetic foot care.
  • Evaluate patient comprehension and progress of their care plan by reviewing clinical and learning outcomes.
  • Maintain open communication and work closely with RFHT and community primary care providers on patient care issues.
  • Create and maintain complete, accurate and timely patient related documentation using the Electronic Medical Records (EMR) or other electronic database such as Excel spreadsheets in accordance with the college standards and regulations.  Exercise reasonable care and caution in protecting confidential and sensitive information.

Education & Specific Job Requirements

  • Bachelor’s Degree in Nursing or College Diploma – Registered Nursing Program required.
  • 3+ years of experience as a health educator preferred.
  • Current registration with the College of Nurses of Ontario, in good standing.
  • Current Membership with RNAO preferred.
  • Completion of Basic and Advanced Nursing Foot Care certificate preferred.
  • Knowledge of wound care, lower limb circulatory diseases, and diabetes management considered a strong asset.
  • Patient centered approach with thorough knowledge of health promotion, health education principles and practices.
  • Ability to function both independently and as part of an inter-disciplinary team.
  • Proficiency in the use of computers and various software applications.
  • Effective organizational, time management and problem solving skills.
  • Excellent communication, critical thinking and interpersonal skills.

Chiropodist

Rapids Family Health Team is seeking an enthusiastic Licensed Chiropodist who is both self-motivated and enjoys working collaboratively with other health care professionals to manage the needs of patients. 

The Chiropodist will perform patient assessment, diagnosis, treatment, health education, and other clinical footcare activities according to the College of Chiropodists of Ontario.

This is a mobile position; travel may be required between Rapids Family Health Team sites as well as throughout the community based on client, community needs and business requirements.

Job Duties:

  • Perform assessment, diagnosis, treatment, health education and other primary clinical care as it relates to foot care, within the full scope of Chiropody practice in accordance with the College of Chiropodists of Ontario.  Including but not limited to diabetic foot care, nail care and wound care.
  • Works with the interdisciplinary team to develop and maintain a full range of chiropody services.
  • Maintain complete, accurate and timely patient records in accordance with the college standards and regulations.  Exercise reasonable care and caution in protecting confidential and sensitive information related to clients.

Qualifications:

  • Chiropody/Podiatry degree from a recognized college/institute or university required.
  • Current valid registration with the College of Chiropodists of Ontario in good standing and without limitations required.
  • Evidence of current professional liability insurance (with no de-listing from any insurance companies) required.
  • Demonstrated clinical and educational skills including assessing client needs, diagnosing routine foot problems, prescribing treatment and providing referrals. 
  • Demonstrated competency in chosen profession; consistently demonstrate knowledge and expertise in professional practice. 
  • Patient-centered approach that incorporates treatment, illness prevention and health promotion.
  • Ability to function both independently and as part of an inter-disciplinary team.

Occupational Therapist

(1 Year contract)

Rapids Family Health Team is seeking an enthusiastic, progressive, self-directed and professional Occupational Therapist with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of clients.

The Occupational Therapist will see patients based on direct referral from the FHT physicians, as well as facilitate groups based on specific populations including but not limited to our Boosting Balance program. Responsibilities will include assessments and intervention related to falls prevention and balance, prescription of assistive devices, cognitive impairment, home safety, functional impairment and system navigation. Due to the collaborative nature of this position, the Occupational Therapist may be required to attend meetings or conduct business at other offices and external locations.

Education and Qualifications:

  • Bachelors or Master’s Degree in Occupational Therapy required
  • 3-5 years of experience as an Occupational Therapist with experience in a primary health care setting preferred
  • Current registration with the College of Occupational Therapists of Ontario, in good standing
  • Demonstrated competence within the full scope of practice of an Occupational Therapist in the provision of assessment, care, interventions, goal setting and documentation of care needs
  • Membership with the Canadian Association of Occupational Therapists and/or Ontario Society of Occupational Therapists is an asset
  • Ontario Ministry of Health and Long-Term Care Assistive Devices Program (ADP) Authorizer status for ambulation aids (ie: walkers), wheelchairs and power mobility an assest
  • Additional training or credentials in chronic disease management, pain management, seniors focused services and home safety, injury prevention, health promotion activities are considered assets
  • Demonstrated clinical reasoning skills and excellent assessment and therapeutic skills
  • Experience in group facilitation and designing, implementing, evaluating programs
  • Ability to work as a self-directed health care professional in an interdisciplinary setting

Registered Practical Nurse (RPN)

Rapids Family Health Team is seeking an enthusiastic, professional Registered Practical Nurse with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of patients. As part of the interdisciplinary team, the Registered Practical Nurse (RPN) will provide medical assistance to physicians, staff and patients while using their superior organization skills to execute procedures that are considered delegated acts, promote patient education and perform general office and telephone duties. The RPN will have knowledge of and maintain complete, accurate, legible and timely patient records using the electronic medical records (EMR - Practice Solutions preferred).

Education and Qualifications:

  • College Diploma – Registered Practical Nursing Program
  • Current registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse
  • Current First Aid and CPR certificates required
  • Demonstrated ability to work effectively as part of a multidisciplinary team
  • Demonstrated competence with the full scope of practice of a Registered Practical Nurse in the provision of assessment, care, medications, interventions and documentation of care needs

Registered Dietitian (RD)

Rapids Family Health Team is seeking an enthusiastic, progressive, self-directed and professional Registered Dietitian with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of clients.

As part of the interdisciplinary team, the Registered Dietitian will deliver nutritional care in accordance with professional practice standards. The Registered Dietitian will deliver a wide range of services including assessment and treatment of individuals, group programs and consultative advice; focusing on chronic disease prevention and management, health promotion and linkages with community resources. Due to the collaborative nature of this position, the Registered Dietitian may be required to attend meetings or conduct business at other offices and external locations.

Education and Qualifications:

  • Bachelor of Applied Science in Human Nutrition (or other undergraduate program approved by the College of Dietitians) and successful completion of a supervised DC Accredited internship or practicum program, Masters Degree in Nutrition considered an asset
  • Additional training or credentials (e.g. Certified Diabetes Educator, motivational interviewing) are an asset
  • Current registration with the College of Dietitians of Ontario, in good standing
  • 2-3 years of experience as a Registered Dietitian in a clinical dietitian role or primary health care setting

Psychotherapist

Rapids Family Health Team is seeking an enthusiastic, professional Psychotherapist with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of patients.  This is a mobile position; travel may be required between Rapids Family Health Team sites as well as throughout the community based on client / community needs and business requirements.

Working in partnership with Rapids Family Health Team professionals to perform patient psychosocial assessments, provide short-term or single session counselling, crisis intervention, education/advocacy, facilitating psycho educational groups and case management.  Performing these roles well will reduce unnecessary office visits and visits to after-care clinics or emergency rooms.

Education and Qualifications:

  • Masters of Psychotherapy degree from a recognized University
  • Current membership and in good standing with the College of Registered Psychotherapists of Ontario
  • Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, other relevant legislation as well as community mental health and addictions resources
  • Experience working as a Psychotherapist working with patients and families providing counselling / psychotherapy
  • Demonstrated knowledge and experience with a variety of crisis assessment and intervention techniques and support strategies for mental health and/or addictions
  • Experience in a multidisciplinary setting will be considered an asset
  • Teaching and experience facilitating treatment groups
  • Efficient computer skills, working knowledge of electronic medical records
