Nurse Practitioner

Contract (12 months), Full Time Hours

Rapids Family Health Team is seeking an enthusiastic Nurse Practitioner who is both self-motivated and enjoys working collaboratively with other health care professionals to manage the needs of patients.  

This is a mobile position; travel will be required between Central Lambton Wyoming and Rapids Family Health Team Access to Care program sites as well as throughout the community based on patient, community needs and business requirements.

Job Duties:

  • Perform independent and comprehensive individual patient health or symptom directed assessments in our acute and episodic Access to Care clinics.
  • Identify and provide appropriate treatment and/or referral for patient's physical and psychosocial health care challenges.
  • Formulate and communicate medical diagnoses.
  • Coordinate appropriate diagnostic tests and procedures as per RN(EC) scope of practice.  Determine the need for and order screening and diagnostic laboratory tests and interpret the results.  Determine the need for, and order and interpret reports of X-rays, ECGs and other diagnostic imaging as needed.
  • Maintain complete, accurate, legible and timely patient records using the accepted CNO standards for documentation.
  • Participate in the development, planning and evaluation of treatment, education counselling and health promotion programs and activities.
  • Exercise extreme care and caution in protecting confidential and sensitive information.

 Qualifications:

  • Bachelor of Science in Nursing with a Primary Health Care Nurse Practitioner program master’s degree or certificate.
  • Current valid registration with the College of Nurses of Ontario as a Registered Nurse Extended Class RN(EC) and evidence of professional liability insurance required.
  • No restrictions on license (ie: can prescribe narcotics according to relevant legislation).
  • Current First Aid and CPR certificates required.
  • Recent and relevant clinical nursing experience practicing in a primary care, emergency, acute and episodic clinic or care settings considered an asset.
  • Demonstrated competence within the full scope of practice of a Nurse Practitioner in the provision of assessment, care, medications, interventions and documentation of care needs.
  • Effective organizational skills, time management skills, interpersonal skills, problem solving skills and communication skills (both written and oral) and ability to work effectively with patients, families, and all members of the inter-professional team.

Registered Dietitian

Rapids Family Health Team is seeking an enthusiastic, progressive, self-directed and professional Registered Dietitian with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of clients. 

As part of the interdisciplinary team, the Registered Dietitian will deliver nutritional care in accordance with professional practice standards.  The Registered Dietitian will deliver a wide range of services including assessment and treatment of individuals, group programs and consultative advice; focusing on chronic disease prevention and management, health promotion and linkages with community resources.  Due to the collaborative nature of this position, the Registered Dietitian may be required to attend meetings or conduct business at other offices and external locations. 

Education and Qualifications:

  • Bachelor of Applied Science in Human Nutrition (or other undergraduate program approved by the College of Dietitians) and successful completion of a supervised DC Accredited internship or practicum program, Masters Degree in Nutrition considered an asset
  • Additional training or credentials (e.g. Certified Diabetes Educator, pediatric nutrition, motivational interviewing) are an asset
  • Current registration with the College of Dietitians of Ontario, in good standing
  • Experience as a Registered Dietitian in a clinical dietitian role or primary health care setting
  • Experience in group facilitation and designing, implementing, evaluating programs
  • Ability to work as a self-directed health care professional in an interdisciplinary setting
  • Aptitude using Electronic Medical Records (EMR)

Occupational Therapist

Rapids Family Health Team is seeking an enthusiastic, progressive, self-directed and professional Occupational Therapist with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of clients.

The Occupational Therapist will see patients based on direct referral from the FHT physicians, as well as facilitate groups based on specific populations including but not limited to our Boosting Balance program. Responsibilities will include assessments and intervention related to falls prevention and balance, prescription of assistive devices, cognitive impairment, home safety, functional impairment and system navigation. Due to the collaborative nature of this position, the Occupational Therapist may be required to attend meetings or conduct business at other offices and external locations.

Education and Qualifications:

  • Bachelors or Master’s Degree in Occupational Therapy required

  • Experience as an Occupational Therapist with experience in a primary health care setting preferred

  • Current registration with the College of Occupational Therapists of Ontario, in good standing

  • Demonstrated competence within the full scope of practice of an Occupational Therapist in the provision of assessment, care, interventions, goal setting and documentation of care needs

  • Membership with the Canadian Association of Occupational Therapists and/or Ontario Society of Occupational Therapists is an asset

  • Ontario Ministry of Health and Long-Term Care Assistive Devices Program (ADP) Authorizer status for ambulation aids (ie: walkers), wheelchairs and power mobility an assest

  • Additional training or credentials in chronic disease management, pain management, seniors focused services and home safety, injury prevention, health promotion activities are considered assets

  • Demonstrated clinical reasoning skills and excellent assessment and therapeutic skills

  • Experience in group facilitation and designing, implementing, evaluating programs

  • Ability to work as a self-directed health care professional in an interdisciplinary setting

  • Aptitude using Electronic Medical Records (eMR)
