Rapids Family Health Team Access to Care is OPEN to all residents Monday thru Thursday 8AM - 4:30 PM and Fridays 8AM - Noon. If you are unable to secure an appointment with your primary care provider for an episodic medical condition (i.e. ear ache, urinary tract infection, etc.) call 519-491-6188 for an appointment with a Nurse Practitioner.
