Type 2 Diabetes and Insulin
Insulin Therapy
One-on-one appointments with a diabetes educator to help you start insulin or adjust your dose(s) to balance your blood sugar control. We will make sure you understand how your type of insulin works, and how to prevent high and low blood sugar levels. Other diabetes-related topics will also be covered based on your interests and needs.
For adults with type 1 diabetes for at least 5 years, and adults with any duration of type 2 diabetes.
Requires referral and dose adjustment directions from your primary care provider (physician or nurse practitioner).
